As a Community Health Worker (CHW) your vital role is bridging the gap between healthcare providers and the community. As a CHW your focus is in improving health outcomes by providing education, support, and resources to individuals and families. Community Health Worker's will work closely with diverse populations to promote wellness, facilitate access to healthcare services, and empower community members to take charge of their health.

Duties/Responsibilities are but not limited to...

  • Educate community members about health resources, services, and programs available to them.
  • Provide support in navigating healthcare systems, including Medicare an Medicaid.
  • Conduct outreach activities to raise awareness about public health issues and available services.
  • Utilize  motivational interviewing techniques to encourage behavior change and promote healthy lifestyles.
  • Assist individuals dealing with addictio by providing counseling and connecting them with appropriate resources.
  • Collaborate with healthcare professionals, social workers, and public health officials to coordinate care for clients.
  • Maintain accurate records of interactions with clients and report on community health trends.
  • Provide screenings to assess clients needs.
  • Promotes health by educating the client regarding prevention and/or treatment of health needs.
  • Provides emotional support, compassion and is sympathetic to the clients needs.
  • Reviews charts on client "no shows" and cancelled appointments.
  • Document and track client's follow-up care for reporting purposes.
  • Complete reporting per scope of work requirements.
  • In conjunction with the patient's direct care provider(s), iniates the care plan and provides educational information related to continuing care requirements.
  • Exemplifies in daily behavior customer service standards of the  organization.
  • Returns clients coorespondance , as needed, in a timely manner.

Requirements/Qualifications:

  • High School Diploma or GED Equivalent.
  • Must have Community Health Worker (CHW) Certificate of Completetion for the State of Nevada.
  • Bilingual (Spanish) is strongly encouraged to apply.
  • Expereince in working with a diverse population.
  • Ability to operate personal computer.
  • Have the ability to perform basic computer software ( Word, Excel, Publisher, Email etc.).
  • Ability to maintain confidentiality of information, client financial and medical information per HIPPA.
  • Must have their own transportation.
  • Must posses a drivers license.
  • Experience or education in nursing, social work, public health, or a related field preferred.
  • Have the ability to sit, stand, walk, stoop, kneel or crouch
  • Have the ability to lift and/or move up to 20lbs.
  • Knowledge of medical terminolog and healthcare systems.
  • Excellent communications with the ability to engage diverse audiences through public speaking.
  • Familiarity with addiction counseling is a plus.
  • Ability to perform outreach activities effectively within the community.
  • Proficiency in motivational interviewing methods to support client engagement.
  • A passion for improving community health outcomes and advocating for individuals needs.

Job Type: Part-Time/ Full-Time

Pay Range: $16-$20

Expected Hours: 20-40 per week

Work Location: Hybrid remote in Las Vegas, NV
