Position Summary

The Care Navigator is a resident- and family-centred role dedicated to supporting individuals and their loved ones through the transition into long-term care at Inverary Manor. This position serves as a key point of contact for residents, families, Continuing Care, and the interdisciplinary team, ensuring admissions, transitions, and care conferences are coordinated with compassion, clarity, and professionalism.

In addition to resident navigation, the Care Navigator holds primary responsibility for overseeing InterRAI assessments, ensuring they are completed accurately, on time, and used meaningfully to inform person-centred care planning, regulatory compliance, and quality improvement.

Working closely with the Director, Clinical Care, the Care Navigator provides clinical and administrative coordination support while reinforcing Inverary Manor’s commitment to dignity, transparency, and high-quality resident experiences. This role is supportive rather than supervisory and does not include formal management of frontline staff. May be required to work on floor in nursing capacity on occasion.

Key Responsibilities

  1. Resident & Family Navigation
  • Act as a primary liaison for residents and families during admission, transition, internal transfers, and end-of-life care
  • Provide clear, compassionate communication to support emotional and logistical transitions into long-term care
  • Serve as a trusted resource for residents and families regarding care processes, care conferences, and available supports
  • Support resident advocacy and always uphold resident-centred care principles
  1. Admissions, Bed Management & Care Conferences
  • Coordinate and support resident admissions and internal transfers in collaboration with the Director, Clinical Care and nursing team
  • Liaise with Continuing Care and external partners to support smooth admissions and transitions
  • Organize, facilitate, and document Resident Care Conferences in compliance with Nova Scotia Long-Term Care standards
  • Ensure documentation related to admissions, transfers, and conferences is accurate, complete, and timely
  1. InterRAI Assessment Oversight
  1. Care Coordination & Interdisciplinary Collaboration
  • Work collaboratively with:
    • Director, Clinical Care
    • Nursing staff (RNs, LPNs, CCAs)
    • Allied health and interdisciplinary team members
  • Support the development, communication, and updating of individualized care plans
  • Promote continuity of care across disciplines
  1. Clinical & Administrative Support
  • Provide clinical and administrative coordination support to the Director, Clinical Care
  • Assist with documentation, reporting, audits, and regulatory follow-up
  • Support preparation for licensing reviews and inspections
  • Navigate electronic health records and digital care platforms
  1. Quality, Safety & Person-Centred Care
  • Support best practices in resident safety, dignity, and infection prevention and control
  • Assist in identifying opportunities to improve care processes and resident outcomes
  • Participate in committees and quality initiatives as appropriate
  • Reinforce Inverary Manor’s commitment to person-centred, relationship-based care

Qualifications & Experience

Required

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing with the Nova Scotia College of Nursing
  • Minimum three (3) years experience in health care
  • Experience working with older adults, including dementia and end-of-life care
  • Strong organizational, documentation, and communication skills
  • Comfort using electronic health records and Microsoft Office

Preferred / Assets

  • Long-Term Care experience
  • Experience completing or overseeing InterRAI assessments
  • Familiarity with Nova Scotia Long-Term Care Standards and licensing processes
  • Experience working within interdisciplinary teams

Inverary Manor is a modern 71-bed long-term care facility located along Cape Breton’s musical coast. The home is organized into six households with common living areas and kitchens, along with private resident rooms and bathrooms.
